Paycheck Discrepancy?
Members claiming payroll discrepancies must submit a Payroll Inquiry Form, pay stub copy and supporting TPR copy to the Payroll Section at Metrotech. Department Orders (DO) define the specific time periods for which OT is reflected in each paycheck. Insure you check DOs prior to filing an inquiry.

Timely 65-2 Notifications to UFA
Members wishing to have information published in a 65-2 must insure that it is received, in writing, at the UFA Recording Secretary's office at least 30 days prior to the event. Sooner if possible. Late submissions may prevent publishing on time. Please make a quick call to the Recording Secretary's office to verify we received it.

Retirement & 20 Year Plaques
With the inordinate amount of retirements since September 11, 2001, the volume of requests for Retirement Plaques has been extremely high. In order to best insure your company member plaques are received in a timely manner we have streamlined the process. A copy of Plaque Request Form is attached (which can also be found in Delegate Manual sect. W-19). Instructions are as follows.

  • Plaques are only for retirement, 20 year and every five year increment after 20 years -- not for promoted members.

  • Delegate must prepare Plaque Request Form (see attached). Name spelling & dates must be accurate.

  • Completed forms must be faxed to UFA mailroom at 212-683-0690. Minimum 60 days notice is recommended.

  • Mailroom generates plaques for signature by President and Recording Secretary.

  • Mailroom mounts, finishes & packages for presentation.

  • Plaques are delivered to your Delegate at the firehouse, Union Meetings or can be picked up at UFA office -- but only after confirming that it is ready.

  • After faxing, call the UFA switchboard and ask for the mailroom or Ann Elizabeth to confirm they have received your Request Form, and again prior to visiting for a pick-up. Following this process is the quickest way to insure prompt service & accurate plaques.

FDNY vs. NYPD Hockey Game in Nassau Coliseum
A reminder: On Saturday, April 5, 2003 at 7:00pm the 30th Annual NYPD-FDNY Hockey Game will be held at the Nassau Veterans Memorial Coliseum. Funds generated support various charities, including the UFA Widows & Children's Fund, the Thomas Elsasser Fund & the UFA Scholarship Fund. $15 general admission tickets can be purchased through Ticketmaster, from team members or at the door.

Support FDNY Memorial at World Police & Fire Games
A 50/50 Raffle and letter was previously distributed to all firehouses by the FDNY Volleyball team, which lost two members on 9-11. The team is sponsoring a 'Tribute & Thank You' exhibit at The Games this summer in Barcelona, Spain, which will be hosted by FDNY members. Hundreds of FDNY athletes participate in The Games semi-annually, along with over 10,000 members from departments around the world. It is the perfect opportunity for remembering our fallen brothers, and for saying thanks for all the support our international fraternal brothers and sisters have given us in our time of need. $100 per raffle. Washingtonville 5 Memorial Golf Outing
2nd Annual: Monday, May 5 at Stony Ford Golf Club, Orange County, NY, 7am breakfast, 8am Tee-off. Golf carts, refreshments, raffles, breakfast, lunch, dinner. $125 per golfer. All proceeds fund a H.S. scholarship and costs of the Washingtonville, NY monument in honor of Gerard Nevins, Mark Whitford, Dennis Devlin, Bobby Hamilton, Glenn Perry, the 5 local FDNY members lost on 9-11.
